When you come for your appointment please remember to obtain and bring the following:
- Drivers License or a valid ID
- Insurance information
- Referral Letter (if required)
- Copies of X-Ray/MRI/CT images and radiology reports
- List of medications (if any)
We know missed appointments are often unavoidable. If you are unable to make your scheduled appointment, please call your doctor's office at least 24 hours in advance. When you call, we will be happy to reschedule your appointment for a more convenient time. In the event of a last-minute cancellation, we still appreciate your call so that we are able to accommodate other patients.
